Opti-Call Aged Care Services Pty Ltd (ABN: 58 613 481 066) If you would like to make a booking for an eye test clinic for 16-18 residents there are three options - 1. Online form (listed below), 2. Email (bookings@opticall.com.au) or 3. Use the online booking system on this website. Select "Clinic Bookings" which is the next option under menu item "Contact Us".
There are three different online Forms available to request Opti-Call to contact your Facility. If you would prefer to email (bookings@opticall.com.au) we will require the email to include the following information:
- Facility name
- Facility address
- Facility telephone number
- Staff members full name and position making the request
- Full name of resident
- Medicare Number and Medicare Ref No i.e. 1, 2, 3 which precedes name on Medicare card
- DOB
- Reason for requested visit
- Book a full days Eye Test Clinic for at least 16-18 Residents who are eligible for their annual eye test - Head Office Action
- Request an urgent glasses repair, glasses replacement or a spare pair of glasses - Dispenser Action
